Love my Etymotics er4p's (http://www.etymotic.com/consumer/earphones/er4.html). With the rubber flange attachments it's basically a complete seal, better than wearing shooter's earplugs ... somewhere around 30-40 dB outside level reduction. Weckl wore them for a gig once when his Shure's crapped out...

I feel like I've felt "that pain" before from playing, although it never progressed to the point where I was limping.

Tweaked my seat height *higher* by ~1" and it stopped.

I've written real-time audio code on Android before, and there's no reason it can't start (effectively) immediately when you hit the button if coded correctly.

Don't know anything about the metronome space under Android, though, so can't recommend one that's well-written. :( Sorry.
